Research on septic shock
Dear Dr. Mallikarjuna,
I am working in Children’s hospital of Philadelphia as a senior research scientist in the department of Immunology and Psychiatry.I am investigating the pathophysiology of septic shock in terminally ill HIV/AIDS children and adolescent patients.
Basically I am a trauma surgeon in Bangladesh and I have completed my PhD from Japan in molecular pathology. When I worked at the department of Casualty Surgery, Dhaka Medical College Hospital, Dhaka, Bangladesh, I saw a large number of patients specially “burn, road traffic crush injuries or post operative ” developed infections very quickly even with very large dose of antibiotics. Many patients died within few days and doctors had no possible explanations for these kind of unfortunate loss of life.
Research on the cause of septic shock is still very controversial. A large group of researchers in the world believe that “endotoxin” from bacteria is the major cause of septic shock. On the basis of that concept, many pharmaceutical companies developed anti-endotoxins which were used in several septic shock clinical trials in developed countries like USA or Germany and Japan. Recently, in a clinical conference in USA, it was shown that despite of highly sophisticated antibiotic therapy with anti-endotoxin, none of the septic shock treatment strategy is working still now.
In my publications on the mechanism and treatment in animal models, I have shown that when 2M concentration goes down to 30% of its original concentration during septicemia, they die immediately. Bacterial proteases activate high-molecular weight kininogen (HK) and release bradykinin which causes severe hypotension. After bradykinin release from HK, it’s called HKa, which stimulates one of the blood cells (monocyte) and release cytokine (IL-1, TNFand IL-6) and chemokines (IL-8 and MCP-1). IL-1 and TNF comes back to monocyte, bind to the receptors (Mac-1)and give signal through signaling pathway p38, JNK, ERK and NFB and expresses tissue factor which is essential for blood coagulation. Bacterial proteases can cause all the phenomenon those occur in septic shock such as DIC, hypotension, tachycardia followed by bradycardia, low peripheral vascular resistance etc.
I like to investigate further on the pathophysiology of septic shock on a noble finding that major triggering molecule of septic shock is bacterial protease and not the traditional concept of endotoxin. The treatment of shock is possible by using a normal blood protein “alpha-2-macroglobulin” which could be a milestone of the development of septic shock treatment in human.
I can organize this work by collaborating hospitals located in Bangladesh if I have very minimum funding.
I am looking forward for your noble investment to a completely new concept of septic shock treatment that can be a noble approach to cure many septic shock people in the world.
